BRODIX® HEAD REPAIR REQUEST FORM

This Head Repair Request Form must be submitted to evaluate your heads and determine if we will accept your repair job before the heads are shipped to us. This form does not guarantee that your heads will be accepted. Allow at least 72 hours for review of this request; multiple requests will delay the process. (Contact us to follow up on this request if you have not received a reply from BRODIX after one week.)

 

Do not send your heads to BRODIX until we have sent you a Repair Order Number. This will prevent you from spending money and waiting for something that is not worth repairing. Only BRODIX® heads are eligible for repair. Your Repair Order Number should be written on the outside of the box when your heads are shipped to us. Also include a note inside of the box with your contact information and return address.

*BRODIX will not order custom parts to assemble your repair heads. Any special parts that are not what we have in stock will have to be sent to us with your repair heads.

**BRODIX may require you to prepay with cashier’s check before repair work is done. A non-refundable $250.00 inspection fee will be required at time of receipt of your heads. This fee will be applied to the cost of the repair after it is completed.

***Any heads sent for repair will become BRODIX® property if the repair bill is not paid within 90 days.
